چکیده

Abstract Background and Aims Sodium-glucose cotransporter 2 (SGLT2) inhibitors have proven to delay diabetic nephropathy (DN) progression on top of renin-angiotensin system (RAS) blockade. This protection is mainly attributed improvement in renal hemodynamics, although direct effects the kidney cannot be ruled out. The present study aimed identify proteins differentially expressed between vehicle-treated mice treated with empagliflozin, ramipril, or their combination, that could help explain protective mechanisms drugs. Method Twelve weeks old db/db were given empagliflozin (10 mg/Kg/day), ramipril (8 combination both drugs during 8 weeks. Vehicle-treated db/m used as controls. Serum glucose, blood pressure, GFR, albuminuria measured at baseline end study. After weeks, euthanized, kidneys serum saved. A differential high-throughput proteomic analysis by mass spectrometry using isobaric tandem tags (TMT labelling) was performed cortex. Results showed increased glycemia whole experiment, reduced glucose. Ramipril treatment decreased pressure. also incipient DN mesangial matrix compared non-diabetic littermates. All treatments expansion albuminuria. Only 13 (false discovery rate <5% Log2FC ≥1 ≤ −1) when comparing vs mice. only identified alone did not produce significant changes proteins. Kidney renin evidently therapy along tubular transporter scaffolding protein MAP17. results further validated through staining concentration measurement. Renal (55 pg/μg (IQR:49–64), 94 (IQR:71–102), 112 (IQR 104–116) groups vehicle, respectively). However, similar combination. Conclusion combined upregulated a mouse model. increase suggests other different from RAS act regulation glomerular hemodynamics arteriolar tone. Moreover, sodium delivery macula densa does inhibit secretion.

Dual Blockade of the Renin-Angiotensin System in Diabetic Nephropathy

OBJECTIVE Many patients with diabetic nephropathy (DN) have levels of albuminuria > 1 g/day and blood pressure >135/85 mmHg, despite antihypertensive combination therapy, including recommended doses of ACE inhibitors, e.g., lisinopril/enalapril at 20 mg daily. We tested the concept that such patients might benefit from dual blockade of the renin-angiotensin system (RAS). RESEARCH DESIGN AND M...

Renin-Angiotensin-Aldosterone System Blockade in Diabetic Nephropathy. Present Evidences

Diabetic Kidney Disease (DKD) is the leading cause of chronic kidney disease in developed countries and its prevalence has increased dramatically in the past few decades. These patients are at an increased risk for premature death, cardiovascular disease, and other severe illnesses that result in frequent hospitalizations and increased health-care utilization. Although much progress has been ma...

Cannabinoid Receptor 1 Blockade Ameliorates Albuminuria in Experimental Diabetic Nephropathy

OBJECTIVE Cannabinoid receptor 1 (CB1) is localized in the central nervous system and in peripheral tissues involved in energy metabolism control. However, CB1 receptors are also expressed at low level within the glomeruli, and the aim of this study was to investigate their potential relevance in the pathogenesis of proteinuria in experimental type 1 diabetes. RESEARCH DESIGN AND METHODS Stre...

The renin-angiotensin system in diabetic nephropathy.

The renin-angiotensin system (RAS) has key regulatory functions for blood pressure and fluid homeostasis. In addition, dysregulation of the system can have maladaptive effects to promote tissue injury in chronic diseases such as hypertension, heart failure, and kidney disease. These actions for the RAS to promote disease pathogenesis are especially apparent in diabetic nephropathy, the most com...
